Dosage Calculation Formulas for Nursing School

Drug math is the one thing that makes nursing students put down their stethoscope and seriously reconsider their life choices.
It's not because it's hard. It's because nobody teaches it with a clear system. You get a formula, a worksheet, and good luck.
This post fixes that. Every formula you need, explained with real clinical examples — the way it should have been taught from day one.
The One Formula That Solves Most Problems
Before anything else, learn this:
Dose ordered ÷ Dose on hand × Volume = Amount to give
That's it. This single formula handles the majority of drug math questions you'll see in nursing school and on NCLEX.
Example:
Order: Amoxicillin 500 mg
On hand: Amoxicillin 250 mg / 5 mL
Formula: 500 ÷ 250 × 5 = 10 mL
Weight-Based Dosing
Used for pediatric patients and many IV medications. The order comes in mg/kg and you calculate based on the patient's weight.
Formula:
Ordered dose (mg/kg) × Patient weight (kg) = Total dose needed
Example:
Order: Gentamicin 2 mg/kg
Patient weight: 70 kg
Formula: 2 × 70 = 140 mg
Converting pounds to kg: Divide weight in lbs by 2.2
154 lbs ÷ 2.2 = 70 kg
IV Flow Rate — Drops Per Minute (gtt/min)
Used when infusing IV fluids with a manual drip set.
Formula:
(Volume to infuse × Drop factor) ÷ Time in minutes = gtt/min
Common drop factors:
- Macrodrip: 10, 15, or 20 gtt/mL
- Microdrip: 60 gtt/mL (always 60)
Example:
Order: 1,000 mL NS over 8 hours, drop factor 15 gtt/mL
Time: 8 × 60 = 480 minutes
Formula: (1,000 × 15) ÷ 480 = 15,000 ÷ 480 = 31 gtt/min
IV Flow Rate — mL Per Hour
Used when programming an IV pump. Much simpler.
Formula:
Total volume ÷ Total hours = mL/hr
Example:
Order: 1,000 mL LR over 8 hours
Formula: 1,000 ÷ 8 = 125 mL/hr
IV Infusion Time
When you know the volume and rate but need to calculate how long the infusion will run.
Formula:
Total volume ÷ mL/hr = Hours to infuse
Example:
500 mL infusing at 125 mL/hr
Formula: 500 ÷ 125 = 4 hours
Concentration Problems — mg/mL
Used when a medication is mixed in a bag and you need to calculate how many mL to give per hour to deliver the right dose.
Formula:
(Ordered dose × Volume of bag) ÷ Total drug in bag = mL/hr
Example:
Order: Dopamine 5 mcg/kg/min
Patient weight: 80 kg
Bag: 400 mg Dopamine in 250 mL NS
Step 1: Convert 400 mg to mcg → 400,000 mcg
Step 2: Find concentration → 400,000 ÷ 250 = 1,600 mcg/mL
Step 3: Calculate dose → 5 × 80 = 400 mcg/min
Step 4: Convert to per hour → 400 × 60 = 24,000 mcg/hr
Step 5: mL/hr → 24,000 ÷ 1,600 = 15 mL/hr

| 1 tablespoon (tbsp) | milliliters (mL) | = 15 mL |
The 3 Steps That Prevent Errors Every Time
- Write out the full equation before calculating — never do it in your head under pressure
- Check your units — if the order is in mg and your hand is in g, convert first before anything else
- Ask yourself: does this answer make sense? — giving 50 mL of a tablet medication is a red flag. Never administer without questioning an unusual answer.
